Community Mental Health Teams (CMHT)

The Community Mental Health Teams offer an integrated service, with both Rochdale Council staff and Pennine Care Foundation Trust staff working together. Each team provides community based interventions and specialist mental health assessments for adults who are aged between 18 and 65.

There are three main Community Mental Health Teams working across the Rochdale Borough, with the additional services below:

Response Hub
This team offers a point of access to mental health services. The Response Hub offers a triage function including completion of mental health assessments and onwards referral to primary and secondary mental health services or adult social care where appropriate.  

Treatment Support Team
This team provides support to people who take certain medications that require administration by a trained mental health professional.

Mental Health Review Team
This team works with individuals who are open to psychiatry and who are in receipt of a package of support through Rochdale Council and who either live in the borough of Rochdale but no longer require intervention from the community mental health team, or who have been placed out of borough, are in receipt of services funded by Rochdale Council and who are open to psychiatry in the area in which they live.
